David Patrick Love, 44, of Lorimor was charged 7:03 p.m. Friday at the Union County Law Enforcement Center for second-degree fraudulent practice and tampering with records.

According to a Creston Police report, Love documented and submitted Medicaid claims in which he personally provided respite care services totaling 290.75 hours to medicaid clients from Feb. 1 to 28, 2011. Medicare paid Love for those claims. However, documentation by the United States Department of Homeland Security, Immigration and Customs enforcement shows Love was actually out of the country during the stated time period of the Medicare claims.

Value of fraudulent claims are $3,664.13.
